In consideration of receiving services provided by Marta Shedletsky, 

 

I hereby declare as follows: 

1. That Marta Shedletsky has informed me, and I am aware, that she is not licensed under the laws of this state to practice any form of medicine. 

2. That she has stated they will neither diagnose nor prescribe for any condition or problem from which I may appear to be suffering.

3. I understand a Yoga Therapy Session is not a substitute for medical treatment.

4. That the said individual has informed me and I understand that no guarantee or promises of cures have or will be made to me.

5. I understand that Yoga Therapy is an educational process for me to learn self-care techniques that I myself deem useful.

6. That I am 18 years of age.

7. I understand Marta's Payment & Cancellation Policy.

 

Payment and Cancellation Policy

For your convenience, Marta Shedletsky offers a secure online payment option at the time of booking.  You can also pay through Venmo @Marta-Gil-2

She knows your time is valuable and hers is too. She asks that you give her at least 24 hours notice if you need to cancel an appointment. 

You can reschedule up to 12 hours without incurring the cancellation fee.

 

The first time you miss an appointment, Marta will make a note in your file. All future missed appointments will incur 50% of the fee for service that was scheduled.
